Platelet-Rich Plasma: The Preparation Is the Treatment
What PRP actually is
Platelet-rich plasma is made from your own blood. A sample is drawn, spun in a centrifuge to separate its components, and the platelet-rich fraction is collected and injected back into the target tissue. Nothing foreign is added.
The rationale is that platelets carry growth factors involved in the body's own repair signalling, and concentrating them at a site of injury may amplify that signalling. That is a plausible mechanism, and it is the honest way to describe it — plausible and partly supported, rather than proven across every application it is sold for.
Because the material is autologous — your own tissue, minimally manipulated — the regulatory position is considerably simpler than for cell therapies, which is one reason PRP is widely available while other regenerative treatments are not. Why two PRP treatments can be nothing alike
This is the part that almost never appears in a clinic's marketing, and it is the single most useful thing a patient can understand. "PRP" describes a category, not a specification. Preparations differ in at least four ways that plausibly change the result.
Platelet concentration. The whole point is concentrating platelets above their level in whole blood. How far above varies dramatically between systems and protocols. A preparation barely above baseline and one several times richer are both sold under the same three letters.
White cell content. Some preparations retain leukocytes and some deliberately remove them. This is not a trivial detail — the two behave differently in tissue, and which is appropriate is debated by application, with leukocyte-poor preparations often preferred inside a joint.
Spin protocol and equipment. Speed, duration and whether a single or double spin is used all change what ends up in the syringe, as does whether a closed commercial kit or an open manual technique is used.
Volume and number of sessions. A single injection and a course of three spaced across weeks are different treatments with different expected outcomes and different total cost.
The practical consequence: comparing two quotes on price alone compares nothing. The question that separates a serious clinic from a casual one is simple — what platelet concentration does your preparation deliver, and how do you know? A clinic that measures it can answer. One that does not will change the subject.
Where the evidence is strongest
Knee osteoarthritis is where PRP has been studied most and where the case is best, particularly in mild to moderate disease. The realistic goal is reduction in pain and improvement in function for a period of months, not regrowth of cartilage. Tendon problems — tennis elbow, patellar and Achilles tendinopathy, plantar fasciitis — are the other area with reasonable support, though results vary by tendon and by how chronic the problem is. In these conditions PRP is generally considered after a proper course of loading rehabilitation has been tried, not instead of it, which is a point clinics that sell injections rarely emphasise.
What is common to both: the effect is gradual, building over weeks, and it is measured in months of relief rather than permanence. Anyone describing an immediate change is describing the anaesthetic, not the PRP.
Where it is thinner
Hair loss. PRP for androgenetic alopecia has genuine supporting studies and is widely offered, but protocols vary and maintenance sessions are required for the effect to persist. It also works best alongside established medical treatment rather than instead of it. Skin and facial rejuvenation. Marketed heavily, supported unevenly. It is reasonable to expect modest improvement in skin quality; it is not reasonable to expect the volume change of a filler or the lift of a procedure. If volume is the goal, that is a different treatment entirely, and if firmness across an area is the goal, collagen biostimulators work on a different mechanism and timeline.
Advanced arthritis. Here the honest answer is negative. If imaging shows bone-on-bone disease and the pain is mechanical, no injection substitutes for joint replacement. What a session actually involves
A treatment takes under an hour. Blood is drawn as it would be for a laboratory test, spun for several minutes, and the resulting fraction is injected — for joints, ideally under ultrasound guidance so the needle lands where it is meant to, which matters more than most patients expect.
Discomfort during and after is normal, particularly in a joint or a tendon, and a temporary increase in soreness for a day or two is common rather than a warning sign. Because the material is your own blood, allergic reaction is not a concern; infection risk is the standard risk of any injection and depends on technique and setting.
You should expect to be advised to avoid anti-inflammatory medication around the treatment, since the proposed mechanism depends on an inflammatory signalling cascade that those drugs suppress. A clinic that gives no aftercare instruction at all is a clinic that has not thought about the protocol.
Most protocols involve a course — commonly two or three sessions spaced weeks apart — with effect assessed a month or more after completion. Plan travel accordingly: a single visit on a short trip may not deliver the intended protocol, and that is worth resolving before you book rather than after.
Cost, and what makes a quote comparable
PRP sits at the accessible end of regenerative treatment, well below cell-based therapy. Within the coordinated regenerative and longevity programme, published from $4,000 USD, PRP is among the lower-cost options, while stem cell protocols start around $4,000. The gap between them is large enough that patients sometimes choose PRP purely on price, which is a poor reason on its own — they are different interventions with different evidence, as the comparison lays out.
To make two quotes comparable, ask each for the same four things: how many sessions are included, whether ultrasound guidance is used, what preparation system and concentration are delivered, and what follow-up assessment is included. Two prices that look far apart usually converge once those are itemised — and occasionally the cheaper one turns out to be a single session of an unmeasured preparation.
Questions worth asking before you agree
- What platelet concentration does your preparation deliver, and is it measured? The answer separates a protocol from a procedure.
- Leukocyte-rich or leukocyte-poor, and why for my case? A clinic with a reasoned answer is thinking about your tissue.
- Is the injection guided by ultrasound? For joints and tendons this changes accuracy.
- How many sessions, over what period, and what is included in the price?
- What does the evidence say for my specific condition? Not for PRP in general — for yours.
- What would make you tell me this is not appropriate? The most revealing question on the list. A clinician who has a clear answer has boundaries; one who does not, does not.
